Applying for WVCHIP is simple and can be done entirely from home. Choose the option that works best for you:
- Online: Visit www.healthcare.gov or www.wvpath.wv.gov to apply.
- By phone: Call the toll-free helpline at 1-800-318-2596 for questions not answered online.
- Paper Application: Download and complete the application (Requires free Adobe Reader) and mail it with the prepaid envelope provided.
If you prefer face-to-face assistance, outreach and enrollment sites are available by county. Staff at these locations can provide applications and help you complete them.
Once your completed application and documentation are ready, mail everything to your local DoHS office.
